Amendment to Promissory Note Maturity Date Extension

Profusa, Inc. has amended its Amended and Restated Promissory Note, originally dated May 31, 2024, with NorthView Sponsor I LLC. The amendment, effective March 20, 2026, extends the maturity date of the note to December 31, 2026. This modification was agreed upon by both the company and the sponsor, as permitted by the original note's terms. All other conditions of the original note remain in full force and effect.
